Clinical Care Coordinator/Registered Nurse

Rosa Malave BSN, RN, CARN has spent the last 5 years working in community health as a clinical care coordinator with patients with substance use disorders and as the primary nurse for the Hepatitis C Program at PCC Community Wellness Center. In addition to these roles, Rosa is serving as a clinic champion to facilitate the implementation of multiple strategies for HIV Pre-exposure Prophylaxis (PrEP) at her organization. Rosa Malave earned her Bachelor of Science in Nursing degree from the University of Illinois Chicago in 2015 and is currently completing a Doctorate in Nursing Practice degree specializing as a Family Nurse Practitioner. Rosa believes in focusing on the whole person, supporting patients by increasing their management skills, and reducing barriers to treatment services. She is passionate about providing patients with thorough, relevant medical information to empower patients to make informed decisions about their medical care. In addition to her nursing career, Rosa enjoys gardening and other outdoor activities.
