This presentation will review recent findings from two large multisite trials (NIDA CTN trial 0097 and 0099) on approaches to starting XR-naltrexone and Buprenorphine safety and effectively in the age of fentanyl and real world settings. Learners will come away with practical guidance and implementation strategies for buprenorphine induction in the emergency room and the rapid approach to XR-naltrexone induction.

describe the risks of precipitated withdrawal during buprenorphine induction in the context of fentanyl based on quantitative data from two large trials.
conduct the rapid approach to withdrawal management to facilitate XR-naltrexone induction.
